AXXESS quickie tour
This is what we cover in office re: AXXESS. It is NOT a substitute for other material presented here.
Admin will provide caregivers training on how to chart in AXXESS.
For Nurses, there are 4 areas you need to know about: The first two are at the top line of your dashboard.
+CREATE: Here you will find Orders and the Incident Log. [There are other things too but these are the most essential.]
MY SCHEDULE: drop down to find your calendar. If you need to change the date/time, do so under the CLIENT "schedule activity". Unless you are deleting a training / fictional chart note, if a client or employee is a no-show, use MISSED VISIT, not delete.
QUICK REPORTS: The Nurse's best friend. Flow sheets, Care Plans, Med lists, e-MAR/e-TAR: it's all here for easy access.
CLIENT: To find medical records under your client, go to
1. Documents (at the top)
2. (edit) just to the right of the client 'photo'. That will take you to their medical info and contacts faster than documents will.
Remember that AXXESS entries are your TIME CARD. Before signing your note, make sure the start and end times of your shift or visit are accurate. We do not require Nurses to have clients sign their entry. We DO ask that charting be completed DURING your shift and not be put off until later.
The flowsheets make charting efficient, and Nurses can 'pull down' a previous assessment to avoid having to enter stable V/S or the patient's baseline repeatedly. The Nurse will enter current VS and any changes since the last assessment.
For Nurse Delegators, make sure you are documenting on the right client and the right med-tech.
* When it comes to 'ticking boxes', it is easy to go too quickly and miss noting something important. Chart mindfully.
